Headers in "append" markdown files

2 things combining here:

  • The level-1 header in append docs is always discarded.
  • It’s usually beneficial to separate language-specific instructions from the generic instructions

I’ll be creating a PR that adds level-2 headers to Arturo’s exercise instructions.append.md files.

There’s also a third thing: I’m lazy about reading the README.
